Annual Report to the ELCA
Congregational Annual Reports—Form A
Annual Reports are expected to be submitted to the ELCA by congregations annually. The ELCA Office of the Secretary receives annual congregation reports (Form A), which are used to update the parochial statistics of the ELCA.

Form A for 2025
The online system for 2025 Annual Report Form A reporting will open in mid-January 2026. There are several changes to the Annual Congregation Report process to note for 2025:
Congregations will only receive a letter mailed in mid-January 2026. The letter will contain details on how to access the online reporting system, including your congregation ID and password. Congregations will not receive any printed resources through the mail. All resources will be posted online at elca.org/about/congregations/annual-congregation-report. The letter will direct your congregation to these resources.
While a customized copy of Form A will not be mailed to congregations, this form is available upon request. Congregations will need to email research@elca.org to receive a copy.
On Tuesday, December 9, 2025, the ELCA churchwide organization sent an email to all congregations describing the Annual Congregation Report process and key dates for the new data year. The email also provided a link to elca.org/about/congregations/annual-congregation-report which includes a generic version of Form A, so congregations that wish to begin compiling their data can do so.
The deadline to submit the 2025 Congregation Annual Report Form A is March 1, 2026.

Annual Report to the Synod Bishop
All 2025 reports are due by March 15, 2026.
Rostered Minister (RM) Annual Reports to the Synod Bishop
All Ministers of Word and Sacrament and Ministers of Word and Service whether active, on leave or on disability from call, or retired are asked to complete and submit an annual report to the synod bishop each year. The online fillable report forms are available through the buttons below. Please choose the report that applies to you, the Report for Rostered Ministers serving under Call from Church or Synod Council or the Report for Rostered Ministers who are Retired, On Leave, Or On Disability.
Synod Authorized Minister (SAM) Annual Reports to the Synod Bishop
All Synod Authorized Ministers are asked to complete and submit an annual report to the synod bishop each year. The online fillable Report for Synod Authorized Ministers is available through the button below.
